Targeted Case Management (TCM) helps children, young adults, and families connect with the services and resources they need to improve their overall wellbeing. At Texas Care, our case managers work closely with families to identify challenges, create goals, coordinate care, and provide guidance throughout the process.
Many families face difficulties that go beyond behavioral or emotional concerns alone. Issues such as school struggles, lack of community resources, unstable living situations, healthcare access, transportation barriers, or family stress can all impact a child’s ability to succeed. Targeted Case Management is designed to help families navigate these challenges and connect them with the right support systems.
Our case managers act as advocates and coordinators, helping families understand available services and ensuring care is organized around the child’s unique needs. This may include communication and coordination with schools, healthcare providers, community organizations, and other support systems involved in the child’s care.
Targeted Case Management helps families coordinate behavioral and mental health services while connecting them with important community resources and support systems. Case managers may assist with school-related support, communication between providers, treatment planning, and identifying barriers that may affect a child’s progress. Services can also include helping families navigate Medicaid and healthcare systems, coordinating care between multiple providers or agencies, and connecting families with resources such as food assistance, housing support, transportation resources, and other community programs.
